Schedule Appointment

1. Send your appointment request to submit a visa or residence permit application to: AddisAbeba.VISA@mzv.gov.cz
(NOTE: You must send your request specifically to the email address AddisAbeba.VISA@mzv.gov.cz. If you send your request to any other email address, your request will be considered a spam and will be ignored.)

2. State the following in the email SUBJECT: "Visa appointment request -" followed by your full name as it appears in the passport (for example: "Visa appointment request - Abebe Hailu Tesfaye")
(NOTE: If you put anything else in the email subject, your request will be excluded and ignored.)

3. In the BODY of your email include ALL the following information exactly in the hereby specified order:

(NOTE: DO NOT include the explanation remarks (*) mentioned above. Those are for your reference ONLY. If any information is missing, your request will be excluded and ignored. If the purpose of stay regarding employee card and seasonal work is not clearly specified, your request will be excluded and ignored.)

4. Attach the following documents in your email: 1) passport data page and 2) document confirming your purpose of travel. Attached documents must be in PDF format ONLY and CANNOT exceed 1 MB in size (each one of them). If your purpose of travel is TOURISM, attach the passport data page ONLY and DO NOT attach any other documents. If your purpose of travel is STUDY, attach letter of confirmation from Czech educational institution in Czech language ("Potvrzení o studiu"), BUSINESS = invitation letter from Czech business partner, CONFERENCE = invitation to conference issued by Czech organizer, EMPLOYEE CARD = document (usually your Work Contract) clearly showing the number under which your work position is registered in the Central Register of Vacancies of the Czech Ministry of Labor and Social Affairs of the Czech Republic ("číslo volného pracovního místa v centrální evidenci volných pracovních míst MPSV ČR"), SEASONAL WORK = Czech work permit ("Povolení k zaměstnání"), SPORT = invitation to sports event issued by Czech organizer, FAMILY = copy of Czech residence permit of the family member living in the Czech Republic, OFFICIAL DELEGATION = copy of a Verbal Note issued by the relevant Ethiopian Ministry.
(NOTE: If you do not provide the requested document specified above or if you attach any other unrequested document/s or if the size of each document exceeds 1 MB, your request will be excluded and ignored.)

5. Send one separate email per each applicant and send one email ONLY. One (1) applicant equals one (1) email! Each email may ONLY contain one appointment request with the exception of 1) families travelling together, 2) official, business or sports delegations. In such situation, one member of family/delegation includes the other members into one email request providing ALL the requested information individually one after another in the body of the email. 
(NOTE: If your email contains more than one applicant and you are not a family/delegation, your request will be excluded and ignored.)

6. Your visa appointment request will be registered in a queue in the order received (by your email's date and time). The embassy does not alter the order of registrations unless the request comes from a preferential group of applicants (usually: official and business delegations, "bona fide" applicants, international conference attendees and students). Appointments are scheduled in the order in which the email request was received, in accordance with the principle of transparency and predictability and based on the current embassy operational capacity. 
(NOTE: If you send more than one identical email visa appointment request, your LAST email request will be considered as valid. All your previous email requests will be ignored and you will be positioned to the end of the queue again.)

7. The following emails will be excluded from the registration and ignored: 1) emails not sent to AddisAbeba.VISA@mzv.gov.cz, 2) emails without ALL the information requested in the body of the email, 3) emails with a subject other than specified in these instructions, 4) emails missing the requested attachments, 5) emails containing attachments that are not requested in these instructions, 6) emails with attachments exceeding 1 MB each, 7) emails containing requests for registration of more than one person, with the exception of families and official/business/sports delegations, 8) emails sent by one applicant more than once.

8. Wait for an email confirmation from the embassy informing you that your registration has been accepted. The embassy will inform you about the appointment date and time ONLY at the moment when there are any new vacant appointments which may take some time (weeks or months depending on the current number of applicants in the queue before you - see the table below).

9. If you do not receive the confirmation email from the embassy within 30 days, please, consider that your email request was excluded due to the fact that the request was not sent according to these instructions. In such a case you may resend your request following these instructions exactly step by step. 
(NOTE: You will NOT receive an email from the embassy in case your request was not sent according these instructions as it is not in the operational capacities of the embassy to correct hundreds of applicants who ignore the rules.)

10. The appointments dates and times cannot be rescheduled! If you fail to arrive for the appointment, you lose the appointment. If you arrive late without notifying the embassy beforehand by email, you lose your appointment as well. For operational and security (!) reasons the embassy will ONLY provide services to those applicants who arrive on time for the previously scheduled appointment. 

11. If it is not realistically possible to accommodate your appointment request taking into account your planned date of departure, number of other applicants currently waiting for appointment in queue in front of you and limited capacities of the embassy, your appointment request WILL BE CANCELED and the embassy will inform you about the cancelation by email. This might happen anytime - at the moment you apply for an appointment or at any time later when embassy reconsiders your request which is done periodically.
