Study

LIST OF REQUIRED DOCUMENTS TO BE PRESENTED BY APPLICANTS FOR LONG-TERM VISA/RESIDENCE FOR PURPOSE OF STUDY

IMPORTANT
- incomplete applications will not be accepted and will be returned to the applicant. Your original visa appointment request will be cancelled and you will have to apply for a new appointment and wait in the queue for the next available slot therefore be careful while preparing your application documents.
- all documents (except for the proof of sufficient financial means) must be presented in Czech language.
- all documents must be presented in the original or as a notarized copy AND one (simple) copy of each document (i.e. two complete sets of documents must be provided).
- all documents (except for the travel document) may not be older than 180 days.
- while submitting the application, an interview with the applicant might be conducted and the written record of the interview will be part of the application. The record will be written in Czech language and must be signed by the applicant and by the Consular Officer.
- in case you do not speak English or Czech, you must bring a certified interpreter with you.

PLEASE, SUBMIT THE DOCUMENTS IN ORDER FROM no. 1 TO no. 14 EXACTLY AS FOLLOWS:

1) 
visa application form ("long-term visa form" or "long-term residence permit form" according the information you have received from your Czech school)

- form must be filled in completely 
- DO NOT attach, staple or glue any photo on the form
- SIGN where indicated 

2)
one color copy of the filled visa application
 
3) 
two passport size photographs
- photos must be identical, recent, in color, size 3,5 x 4,5 cm, entire face must be clearly visible, with white background and in a good print quality on high quality photo paper, made at professional photo studio; do not print photos yourself or the application will be returned to you as unacceptable (!)

4)
passport
- original (!)
- validity must exceed intended stay by at least 3 months
- must be issued within the previous 10 years
- must contain at least two blank pages

5) 
two color copies of the data page of the passport
 
6)
document confirming the purpose of stay
- written confirmation of enrolment into studies issued by the Czech school (Acceptance Letter = "Doklad o přijetí ke studiu")
- this document must be written in the Czech language and must be submitted in original (scanned or simple copies are not acceptable)

7) 
one color copy of the document confirming the purpose of stay
 
8)
proof of accommodation
- document issued by the Czech school, verifying the accommodation for the duration of the stay OR a written confirmation of the owner of the apartment/house with a notarized signature (by a Notary Public in the Czech Republic) OR rental/lease contract, sub-letting contract, accommodation agreement or an agreement with similar content
- original or notarized copy in Czech language of any of the mentioned documents must be submitted

one color copy of all the documents submitted as proof of accommodation
 
10)
documents confirming sufficient financial resources
- it is necessary to prove the availability of financial funds that are 15 times the amount of the existential minimum (currently CZK 3.130 per month) for the first month of the stay, AND double (2x) the amount of the existential minimum for every following month of the stay
- for example, the required miminum for 11 months of stay would be (1 x 15 x 3,130) + (10 x 2 x 3,130) = 35 x 3,130 = CZK 109.550, i.e. approx. ETB 700.000 or USD 5,500. The equivalent amount in US dollars (USD) or Ethiopian birrs (ETB) depends on the current exchange rate.

Funds to cover the stay in the Czech Republic shall be proven by ONE of the following means:
(A) "debit/credit card" AND applicant's "bank account statement", OR
(B) "debit/credit card" AND "bank reference letter", OR
(C) "debit/credit card" AND "letter from a family member confirming the financial support" AND family member's "bank account statement", OR
(D) "debit/credit card" AND "financial guarantee"
- SEE BELOW for specifications of the documents to be submitted:

"debit/credit card":
- only a currently valid, internationally recognised debit OR credit card will be accepted
- name of the cardholder must match name of the visa applicant. Anonymous or nameless cards are not acceptable.
- debit/credit card must be issued by the same bank for which the "bank account statement" (or the "bank reference letter") is submitted
- make sure that the applicant's name, the name of the issuing bank, the card validity date and the card number digits are clearly visible; the card security code (CVV number) may be hidden for security reasons

"bank account statement":
- must cover the last 3 months
- only checking AND/OR savings bank accounts are acceptable; credit accounts are not acceptable
- bank account must be kept in the applicant's name (or in the name of the sponsoring family member); joint account ownership is permitted
- only original statements are acceptable; printshots of account details from online banking (made by the applicant or by the bank clerk) are not acceptable

"letter from a family member confirming the financial support":
- original confirmation of a family member's financial support must be provided with a notarized signature AND, if not written in Czech, with a certified translation into the Czech language
- family member's bank account statement must be presented (see "bank account statement" for specifications).
- two (2) copies of a valid ID/passport that confirms full name and and date of birth of the sponsor

"bank reference letter":
- original letter from the bank must refer to bank account(s) kept in applicant's name 
- document must clearly indicate the total balance that the applicant is free to use during the stay in the Czech Republic
- document must clearly indicate the issuing bank, and the signature, name, position and contact details of the issuing bank clerk
- if not written in Czech, the document must be provided with a certified translation into the Czech language

"financial guarantee":
- original confirmation issued by a state authority, or by the Czech school, confirming that all of the costs of student’s stay in the Czech Republic will be fully covered
- if not written in Czech, the document must be provided with a certified translation into the Czech language

one color copy of all the documents confirming sufficient financial resources

12)
criminal history record
- Ethiopian Criminal History Record that must be superlegalized and must be accompanied by verified translation into Czech language
- if the applicant lived in any other country besides Ethiopia for 6 months or longer in the previous three (3) years, an Excerpt from Criminal History Record issued by that country must also be submitted with the required legalization/apostille and verified translation into Czech language

13)
one color copy of the criminal history record
 
14) 
visa application fee
- visa fee must be paid in cash in EUR
- current visa fee is aprox. 110 EUR (you must bring the exact amount!)
- if applicable, consular fee for superlegalization of the Ethiopian Criminal History Record is aprox. 50 EUR
- if applicable, consular fee for legalization of translation of the Ethiopian Criminal History Record is aprox. 25 EUR per each page of the translation

15)
travel medical insurance
- the exact conditions of the travel medical insurance will only be specified in case the visa is approved
- travel medical insurance is the only required document that is to be submitted only after the Embassy receives instruction from the Ministry of Interior to issue the visa and not at the time of visa application submission
