Czech Criminal Record Certificate
An extract from Criminal Records Register of the Czech Republic may be obtained through the Embassy of the Czech Republic in Oslo.
An application for an extract from the Criminal Records Register of the Czech Republic may be submitted in person at the headquarters of the Criminal Records Register (Legerova 1877/7, Prague 2) or at our embassy upon previous appointment.
1. An application for an extract from the Criminal Records Register may be submitted in person during the official hours of the consular section of the Embassy of the Czech Republic in Oslo. It is also possible to apply through an authorized representative based on a power of attorney (officially certified and, if applicable, officially translated into the Czech language). The administrative fee of CZK 400 is paid by card.
A valid identity document (ID card or passport) must be presented with the application.
If the applicant is a foreign national (not a Czech citizen) who is not registered in the Population Register and some of the verified data are missing from the identity document (e.g. maiden name or place of birth), the missing data must be documented by an additional document (birth certificate, marriage certificate, etc.) with a higher level of authentication and, if applicable, an officially certified translation. In such cases, please contact us before submitting the application by phone at +47 2212 1036 or by e-mail: oslo.consulate@mzv.gov.cz.
The Criminal Record Extract is issued in the Czech language and, upon request, also with a so‑called multilingual standard form pursuant to Regulation (EU) 2016/1191 of the European Parliament and of the Council of 6 July 2016 for use in EU countries. Issuance of the multilingual form is subject to a separate fee of CZK 300.
2. If you need a Criminal Record Extract provided with higher authentication (apostille), a written application must be submitted. The administrative fee for the issuance of an apostille is CZK 600.
