APPLICATION FOR CZECH PASSPORT FOR A CHILD (0-17 YEARS OF AGE)
SUBMITTED BY ANOTHER HOLDER OF PARENTAL RESPONSIBILITY

 

WHO ATTENDS THE APPOINTMENT?

The care holder/representative of the care-holding institution is required to be present to submit the application. The child should come to the appointment if he/she is able to sign his/her surname.

Please keep in mind that the holder of parental responsibility must be officially appointed by court and must be stated in the lawful care order.

WHAT IS THE DEADLINE?

Czech law stipulates the deadline of 120 days for passport delivery.

WHAT DOCUMENTS ARE NECESSARY FOR SUBMITTING THE APPLICATION?

  • Czech birth certificate of the child
  • Proof of Czech citizenship of the child:
    • Valid passport or national ID of the child, or
    • Valid Certificate of Czech Citizenship issued in last 12 months
  • Passport photo of the child, size 5x5 cm (more info here)
  • Valid personal ID (passport) of the applicant (representative of care-holding institution)
  • If the holder of parental responsibility stated in the court order is an institution, besides his/her own passport, the applicant must present a proof that he/she is truly the representative of this care holding institution (name badge, official letter from the institution, etc.)
  • Care order* + Certificate* referred to in Article 39 of Council Regulation (EC) No. 2201/ 2003 of 27 November 2003 (1) concerning judgements in matrimonial matters (for more information please see here and here. You can request this certificate by the respective court that issued the care order.
  • The fee for the Czech passport application in cash (according to current consular rates).

 

WHAT PRINTED AND FILLED-OUT FORMS ARE TO BE PRESENTED DURING THE APPOINTMENT?

  1. Personal information (personal data of the applicant)
  2. Proclamation (personal data of children older than 15 or the parent)
  3. Proclamation for children (only for children younger than 14 years of age)
     

The applicant who provides false information is a possible subject to prosecution.

NOTE:

The Consulate General of the Czech Republic in Manchester is a submitting institution, however, the respective Czech authorities in the Czech Republic process and evaluate the application.
