passport application

This chapter provides more information about obtaining a passport for a looked after child.
 

 

A passport is the primary travel document for travel abroad. It i salso used as proof of citizenship and personal ID.

If a child in your care has a valid passport that is due to expire within six months, you can apply  for a new one.

Below is a list of documents you will need to apply for a child aged 0-17.

  1. printed and filled out forms:
  2. original of the Care order with an apostille and court-appointed translation into Czech (only choose someone in bold here),
  3. original of the child’s Czech birth certificate,
  4. valid passport/national ID of the holder of parental responsibility, or the representative of the care holding institution,
  5. a work badge to prove you are a representative of the care holding institution,
  6. passport/national ID or Certificate of Czech citizenship of the child,
  7. proof of the child acquiring a foreign citizenship with an apostille and court-appointed translation into Czech, if relevant,
  8. The holder of parental responsibility/representative of the care holding institution is required to attend the appointment in person together with the child to submit the application.

Passport photographs with be taken at the appointment.

Once submitted, the application will be sent to the Czech Republic based on the child’s permanent residence address. The process of issuing the passport can take up to 4 months, it is, however, usually ready within 2 months. It is not possible to pay extra for the process to be expedited.

You can choose to pick up the new passport at our Consulate, or in the Czech Republic in the city where the child’s last permanent address was. It is not possible to send it by post.

The validity of the passport is always 10 years for persons aged 15+, and 5 years for children aged 0-14.
