Emergency travel document

This chapter provides more information about obtaining an emergency passport for a looked after child.

 

An emergency passport is issued to Czech citizens for an immediate one-way journey to the Czech Republic, and only in certain cases proven to be emergencies. In case of looked after children, it can be issued once there is a Court order placing the child into care of a relative who resides in the Czech Republic.

Proof of the situation being an emergency will be required before any appointment is made for the child. A copy of a Court order placing the child in care of a relative in the Czech Republic is the most commonly provided proof.

An emergency passport will not be issued as a “replacement passport” in the case of a child residing in the UK with no plans to be returned to the Czech Republic, wishing to go on holiday. You must apply for the standard documents in these cases.

Below is a list of documents you will need to apply for a child aged 0-17.

  1. printed and filled out forms:
  2. original of the Care order with an apostille and court-appointed translation into Czech (only choose someone in bold here),
  3. original of the child’s Czech birth certificate or foreign birth certificate with apostille and court-appointed translation into Czech,
  4. valid passport/national ID of the holder of parental responsibility, or the representative of the care holding institution,
  5. a work badge to prove you are a representative of the care holding institution,
  6. any previous passports/national IDs or Certificates of Czech citizenship of the child,
  7. proof of the child acquiring a foreign citizenship with an apostille and court-appointed translation into Czech, if relevant,
  8. two passport sized photographs of the child,
  9. flight tickets within 10 days from the appointment.

The holder of parental responsibility/representative of the care holding institution is required to attend the appointment in person together with a child aged 6+ to submit the application.

The emergency passport is issued for a very limited period of time covering a single one-way journey to the Czech Republic. The validity of the document is usually 10 days, during which you must get to the Czech Republic.

Once in the Czech Republic, this emergency passport must be returned to the passport office of the city where the child will live.
