If looked after child's passport or national ID has been lost or has expired, application for a new Certificate of Czech citizenship must be made. Only after receiving this Certificate a new passport for the child may be applied for.
The Certificate of Czech citizenship is proof that the child you have in care is a Czech national. It is necessary to apply for it whenever a child does not have a passport or national ID (either it has expired or has been lost or in case you are registering the child’s birth).
please find below the list of documents you will need to apply for a child aged 0-14:

printed and filled out forms:
- NB: if you don’t have the child’s Czech birth certificate available: Application for a duplicate
- original of the Care Order with an apostille and translation into Czech by a court-appointed translator (only choose someone in bold here)
- original of the child’s Czech birth certificate
- valid passport/national ID of the holder of parental responsibility, or the representative of the care holding institution
- a work badge to prove you are a representative of the care holding institution
- any expired passports/national IDs or Certificates of Czech citizenship of the child
- proof of the child acquiring foreign citizenship with an apostille and translation into Czech by a court-appointed translator, if relevant
- any other relevant documents relating to the child (change of name deed etc.)
The holder of parental responsibility/representative of the care holding institution is required to attend the appointment in person to submit the application. The child does not need to attend the appointment.

Once submitted, the application will be sent to the Regional Authority in the Czech Republic based on the child’s permanent residence address. The process of issuing the Certificate can take 2 to 6 months.
We will email you once we receive the Certificate of Czech citizenship. The next step is to apply for a passport for the child.
If you have a previous Certificate of Czech citizenship for the child, check its validity – it is only valid for one year from its date of issue. if it has expired, you must apply for a new one before a passport application can be made.
