SPECIFIC SITUATIONS – NAME AND SURNAME OF THE CHILD
The parents are married – which surname will be in the child’s Czech birth certificate?
If the child was born when the parents were married, the parents’ shared surname will be used. If parents have different surnames, the surname, which was determined by the marriage agreement, will be used. Enclosed with a written agreement of a child 15 years and older, i.e. the child will join his/her parents at the appointment.
The parents are not married and have different surnames - which surname will be on the Czech birth certificate of the child?
The Czech birth certificate will contain the surname of one of the parents based on the parents’ agreement or a court judgement regarding fatherhood. Both parents have to sign the agreement (only one parent is sufficient if the other one is not stated in the child’s birth certificate). Children of 15 and older have to come to the appointment together with the parents to sign the form; if not, the parents have to present the child’s verified signature.
Can the daughter’s surname be registered in the male form (without “–ová”)?
Yes, if the child meets at least one of these requirements, if:
she is Czech and has or will have permanent residence abroad, or
she is Czech and one of her parents is a foreigner, or
she has another citizenship.
In those cases, fill out the form Registering the surname of the child in the male form (without „ová“)
This form must be signed by both parents (or only one if the other is not stated in the birth certificate of the child) and a child of 15 and older sign it (if the child’s signature is verified, then they do not need to come).
