WHAT DOCUMENTS ARE NECESSARY TO SUBMIT THE APPLICATION?
- A Birth certificate of the deceased person
- A Marriage/divorce certificate of the deceased person*
- A Death certificate*
- Birth certificates of the parents (if available)
- Marriage/divorce/death certificates of the parents (if available)
- Earlier Certificates of Czech Citizenship of the deceased person and of their parents (if available)
- Proof of acquiring different citizenship* (if they ever acquired different citizenship)
- Evidence of adoption* (if they were adopted)
- Other relevant documents (proof of change in name* etc.)
- The application fee (in accordance with current consular rates).
- Printed and filled out forms :
*British documents must bear an apostille and must be translated into Czech by a sworn/court-appointed translator. Court-appointed translators, whose translations do not need any further verifications, are highlighted.
