CERTIFICATE OF CZECH CITIZENSHIP (DECEASED)
 

WHAT DOCUMENTS ARE NECESSARY TO SUBMIT THE APPLICATION?

  1. A Birth certificate of the deceased person
  2. A Marriage/divorce certificate of the deceased person*
  3. A Death certificate*
  4. Birth certificates of the parents (if available)
  5. Marriage/divorce/death certificates of the parents (if available)
  6. Earlier Certificates of Czech Citizenship of the deceased person and of their parents (if available)
  7. Proof of acquiring different citizenship* (if they ever acquired different citizenship)
  8. Evidence of adoption* (if they were adopted)
  9. Other relevant documents (proof of change in name* etc.)
  10. The application fee (in accordance with current consular rates).
  11. Printed and filled out forms :
    1. Personal information
    2. Questionnaire and proclamation
    3. Application for Certificate of Czech Citizenship
       

*British documents must bear an apostille and must be translated into Czech by a sworn/court-appointed translator. You can see the list of translators who provided us with their contact information on our website – click here. Court-appointed translators, whose translations do not need any further verifications, are highlighted.


If you are submitting a document issued in another country, check the verification and translation requirements for the specific country.

 

The applicant who provides false information is a possible subject to prosecution.
