CERTIFICATE OF CZECH CITIZENSHIP (0 – 14)

Only one of the parents is required to be present to submit the application. A child younger than 15 years of age does not need to attend the appointment.

WHAT DOCUMENTS ARE NECESSARY TO SUBMIT THE APPLICATION?

  1. Valid ID of the parents (an ID card, a passport or a driving license)
  2. Czech birth certificate of the child
  3. Evidence of acquiring other citizenship,* if applicable
  4. Evidence of adoption* (if the child is adopted)
  5. The application fee (in accordance with current consular rates).
  6. Printed and filled out forms:
    1. Personal information
    2. Questionnaire and proclamation for persons younger than 15
    3. Application for Certificate of Czech Citizenship
       

*British documents must bear an apostille and must be translated into Czech by a sworn/court-appointed translator. You can see the list of translators who provided us with their contact information on our website – click here. Court-appointed translators, whose translations do not need any further verifications, are highlighted.

If you are submitting a document issued in another country, check the verification and translation requirements for the specific country.

The applicant who provides false information is a possible subject to prosecution.
