CERTIFICATE OF CZECH CITIZENSHIP (0 – 14)
Only one of the parents is required to be present to submit the application. A child younger than 15 years of age does not need to attend the appointment.
WHAT DOCUMENTS ARE NECESSARY TO SUBMIT THE APPLICATION?
- Valid ID of the parents (an ID card, a passport or a driving license)
- Czech birth certificate of the child
- Evidence of acquiring other citizenship,* if applicable
- Evidence of adoption* (if the child is adopted)
- The application fee (in accordance with current consular rates).
- Printed and filled out forms:
*British documents must bear an apostille and must be translated into Czech by a sworn/court-appointed translator. Court-appointed translators, whose translations do not need any further verifications, are highlighted.
If you are submitting a document issued in another country, check the verification and translation requirements for the specific country.
The applicant who provides false information is a possible subject to prosecution.
