Only one of the parents is required to be present to submit the application. A child younger than 15 years of age does not need to attend the appointment.

WHAT DOCUMENTS ARE NECESSARY TO SUBMIT THE APPLICATION?

1. Valid ID of the parents (an ID card, a passport or a driving license)
2. Czech birth certificate of the child
3. Evidence of acquiring other citizenship,* if applicable
4. Evidence of adoption* (if the child is adopted)
5. The application fee (in accordance with current consular rates).
6. Printed and filled out forms:
   1. Personal information
   2. Questionnaire and proclamation for persons younger than 15
   3. Application for Certificate of Czech Citizenship
       

*British documents must bear an apostille and must be translated into Czech by a sworn/court-appointed translator. The applicant who provides false information is a possible subject to prosecution.
