Applicants residing in Shanghai, Zhejiang, Jiangsu or Anhui who would like to apply for a long-term visa or long-term residence permit (except for Employee Card), are required to send an e-mail to shanghai.visa@mzv.gov.cz and request an appointment.
E-mail requests for appointment are processed in the order in which they are received.
The Consular Section is not authorized to interfere with, modify, or influence the order of applicants' registrations in any way.
Requests to change the appointment date or the type of visa cannot be accommodated, and the Consular Section will not respond to such requests. If an applicant is unable to attend the Consular Section on the scheduled date, they must submit a new standard request.
Requirements for appointment requests:
- One applicant must submit one e-mail. An exception applies to minors whose applications are submitted together with a parent or legal guardian. In such cases, the parent or legal guardian must indicate this intention in their e-mail request.
- Registration e-mails that cannot be replied to will be excluded from registration.
- The e-mail must be written in English or Czech.
- The e-mail request must include following information:
| Personal Data |

| Scan or photographs of the following documents in PDF, JPG or TIF: |

Without these information and attachments your request will not be further processed.
Applicants with a confirmed appointment for a long-term visa or long-term residence permit may submit their application in person on the scheduled date and time at the Consulate General of the Czech Republic in Shanghai.
Upon receiving the e-mail confirmation of your appointment, please print it and bring it with you.
We strongly recommend that you send your e-mail request for an appointment only after all required application documents have been completed and are ready for submission to the Consulate General of the Czech Republic in Shanghai.
