A) WORK / EMPLOYMENT
1. Application Form. To download form.
2. Travel document / Passport. Passport should be valid for at least 90 days after the intended departure from Czech Republic.
3. Two (2) recent photographs.
- Photo size should be 3.5 x 4.5 cm, with a light-colored background.
- One photograph must be pasted/glued on the designated space on the upper right side corner of the first page of the application form.
- The extra photograph must be paperclipped on the upper left side corner of the first page of the application form.
- Both photographs should be signed on the back.
4. Document confirming the purpose of the stay in the territory of the Czech Republic, namely:
- original or a certified copy of the employment permit (Rozhodnutí o povolení zaměstnat cizince) issued by a labour office in the Czech Republic. This document shall also support the application of a partner/member, the statutory body or a member of a statutory or other body of a company who performs for the company day-to-day tasks resulting from the object of business activities of the company as the legal entity, or the application of a member of a cooperative or a statutory or other body of the cooperative who performs for the cooperative day-to-day tasks resulting from the object of business activities of the cooperative as the legal entity; or¨
- or the reference number and the address of the labour office in the Czech Republic which reviews your application for employment permit; or
- a document proving the fulfilment of the condition stipulated in Section 98 of Act No. 435/2004 Coll. on employment, i.e. that the alien's employment does not require an employment permit issued by a labour office in the Czech Republic;
5. Confirmation about accommodation "Potvrzení o zajištění ubytování" and also a document confirming the availability of accommodation for the period of stay in the territory of the Czech Republic, i.e.:
- an original or a certified copy of an accommodation, lease or sublease agreement; or
- a certificate issued by an individual or a legal entity providing accommodation to you, including a certificate issued to the alien by the party which accommodates him (pursuant to Section 100(e) of Act No. 326/1999 Coll.);this certificate must be officially verified; or
- an authenticated Invitation form in case of a temporary stay; or
- an original or a certified copy of an excerpt from the real estate cadastre, if you own real property;
Accommodation can only be secured in a building intended for living, accommodation or recreation. The accommodation provider is obliged to ensure to a foreign national that the floor area of a room for accommodation must be at least 8 m2, if a single person is accommodated, 12.6 m2 if two persons are accommodated; 5 m2 are added to the floor area per every further accommodated person.
6. An excerpt from the Penal Register (or another similar document) issued by the state of your nationality – for Filipino nationals, the NBI Clearance.
- The NBI Clearance must not be older than six (6) months at the time of application.
- The NBI Clearance without thumbprint will not be accepted.
- The NBI Clearance must be authenticated by the Department of Foreign Affairs, Manila (DFA) and superlegalized by the Czech Embassy in Manila.
- The document must be translated into the Czech language and the translation must be verified. Corresponding fees apply. The translation and verification may be done either through an official translator.
7. Criminal Record Clearance from other states. In case the applicant has stayed in another state for an uninterrupted period of at least six (6) months within the past three (3) years.
The document must be authenticated, superlegalized (unless stipulated otherwise by an international treaty that is binding for the Czech Republic)and translated into Czech in the respective country.
8. Medical report certifying that the applicant does not suffer from a serious disease (if requested by the Embassy of the Czech Republic),
9.Upon confirmation of visa issuance, the following documents must be submitted for the actual issuance of the visa:
- Insurance Coverage and Confirmation of Payment – valid from the date of arrival in the territory until such time that the applicant is covered by public health insurance (employment start date).
- Employment contract or the Statement of the employer ("Prohlášení zaměstnavatele") confirming start date of employment.
*Official documents must not be older than 180 days from date of issuance at the time of application.
* For those traveling to the Czech Republic for work / employment, it is recommended that you contact the Philippine Overseas Employment Administration (POEA) for details on required documentation, if any.
* Photocopies of the required documents also need to be enclosed with the long-term visa application upon submission.
B) BUSINESS / ENTREPRENEURSHIP

4. Document confirming the purpose of the stay in the territory of the Czech Republic, namely:
- a copy of an excerpt from the Trades Register or a certificate issued by a Trades Licensing Office documenting that the alien meets the general and the special conditions for operation of a trade (Section 46 of Act No. 326/1999 Coll. and Section 47(7) of Act No. 455/1991 Coll. on trades); or
- in case of an application presented to the Embassy of the Czech Republic, include in the Application for visa for a stay over 90 days the reference number of the trade notification or of the trade licence application filed with the relevant Trades Licensing Office of the Czech Republic and the address of the Trades Licensing Office where you have filed the trade notification or applied for the licence; or
- a copy of an excerpt from the Commercial Register, with an exception stipulated in Section 21(5) of Act No. 513/1991 Coll., the Commercial Code; or
- a copy of a certificate of registration in the self-employed farmers register, issued by the municipal office;
5. Funds to cover the cost of stay in the territory:
- a bank account statement maintained in the name of the foreign national of which it is obvious that the foreign national can make use of these funds during his/her stay in the Territory of the Czech Republic, or
- any other document on financial security (e.g. an invitation authenticated by the Police), or
- a valid internationally recognized payment card which the foreign national will be obliged, upon request, to supplement with a bank account statement maintained in the name of the foreign national of which it is obvious that the foreign national can make use of these funds during his/her stay in the Territory of the Czech Republic.

9. A document proving that his/her monthly income and the income of people assessed together with him/her is not lower than the sum of the minimum subsistence and costs of housing;
If he/she is a self-employed person: a confirmation of relevant authorities proving he/she has no tax arrears and social security insurance and contribution to the state employment policy arrears;
If he/she stays in the Czech Republic as a statutory body or a member of the statutory body of a trade company or a coop: a confirmation that such company/coop has no tax arrears and social security insurance and contribution to the state employment policy arrears;
Upon request, an income tax payment assessment.

- if you are a child born outside the territory of the Czech Republic – a certified copy of the birth certificate;
- if you are another family member – a document indicating the family relationship, e.g. elderly parents, registered partnership between persons of the same sex, common-law spouses;
5. Funds to cover the cost of stay in the territory:
- a bank account statement maintained in the name of the foreign national of which it is obvious that the foreign national can make use of these funds during his/her stay in the Territory of the Czech Republic, or
- any other document on financial security (e.g. an invitation authenticated by the Police), or
- a valid internationally recognized payment card which the foreign national will be obliged, upon request, to supplement with a bank account statement maintained in the name of the foreign national of which it is obvious that the foreign national can make use of these funds during his/her stay in the Territory of the Czech Republic.

4. Document confirming the purpose of the stay in the territory of the Czech Republic, namely:
- a certified copy of a document on enrolment for studies issued by the relevant school or a proof of study issued by a legal entity pursuant to Section 64 of Act No, 326/1999 Coll.:
- a certified copy of a document issued by a local university in case of participation in a language and vocational preparation for the study in an accredited university study programme organized by a public university, or a participation in a language and vocational training organized as a part of an EU programme or under an international treaty;
- a certified copy of a document issued by the Ministry of Education, Youth and Sports of the Czech Republic (“MEYS CR”) in case of participation in consultations and exchange of experience, in an exchange study stay and in artistic, methodological and other specialized courses and educational events;
- a certified copy of a document issued by a local university, by the Academy of Science of the Czech Republic, by a local secondary school or vocational college, vocational training centre, a special school or an organization accredited by the MEYS CR (www.msmt.cz), in case of participation in a practical vocational training designated to obtain practical professional experience;
- a certified copy of a document issued by the MEYS CR in case of vocational practical training and volunteer service of youth designated for obtaining practical professional experience, which takes place in a domestic host organization, provided that it is held as a part of an EU project, programme or incentive and is organized or coordinated by a local organization accredited by the MEYS CR for such purpose;,
- a certified copy of a document issued by the MEYS CR in case of participation in exchange of experience and study stays of persons responsible for education and human resources development, provided that such activities are performed as a part of EU programmes or initiatives or under international treaties;
5. Funds to cover the cost of stay in the territory:

- any other document on financial security (e.g. an invitation authenticated by the Police), or
