REGISTRATION OF BIRTH BY OTHER HOLDER OF PARENTAL RESPONSIBILITY
WHO ATTENDS THE APPOINTMENT?
The applicant (holder of parental responsibility) attends the appointment (for children between 0 to 14 years of age). If the child is older than 15, he/she must also attend the appointment alongside the applicant.
WHAT DOCUMENTS ARE NECESSARY FOR SUBMITTING THE APPLICATION?
- British birth certificate of the child*
- Care order* + Certificate* referred to in Article 39 of Council Regulation (EC) No. 2201/ 2003 of 27 November 2003 (1) concerning judgements in matrimonial matters (for more information please see here and here). You can request this certificate by the respective court that issued the care order.
- Passport or national ID of the care holder representative of the care-holding institution
- A badge or other proof that the representative represents the care-holding institution
- Czech or Czechoslovak birth certificates + passports of the parents
- If the parents were not single when the child was born (i.e. they were married or divorced or widowed), the respective proof of this marital status is needed (e.g. Czech marriage certificate, Czech divorce certificate; Czech death certificate)
- If the child or the child’s parents have acquired further citizenship(s) – it is necessary to provide evidence* of said citizenship (for example a Certificate of Naturalization* etc.)
- The application fee for registry of birth in cash (according to current consular rates). Please bring the exact amount and be advised that further charges may apply.
*British documents must bear an apostille and must be translated into Czech by a sworn/court-appointed translator. - Personal information:
- For children younger than 15 years of age - personal information of the care holder
- For children 15 and older - personal information of the child
- Application for registering a birth
- Application for the certificate of Czech citizenship
- Declaration and Questionnaire for children younger than 15 years of age
- Declaration and Questionnaire for citizens older than 15 years
HOW LONG DOES IT TAKE?
Registration of birth usually takes a few months, sometimes even up to half a year.
NOTE
The Consulate General of the Czech Republic in Manchester is a submitting institution, however, the respective Czech authorities in the Czech Republic process and evaluate the application.
The applicant who provides false information is a possible subject to prosecution.
