If your relative has passed away abroad, you must notify the Special Registry Office in Brno as soon as possible. This authority will issue a Czech death certificate.
If you decide to apply for registration of death at our office, you will need to submit the following documents and evidence:
- printed and completed forms:
- valid passport or valid ID card of the notifier (or its digital equivalent in the eDoklady application)
- British (foreign) death certificate with an apostille and translation into Czech by a court-appointed translator. Please see the list of translators who provided us with their contact information in the list of translators page (court-appointed translators, whose translations do not need further verification, are highlighted)
- documents proving Czech citizenship of the deceased (valid passport or ID card, certificate of citizenship)
- birth certificate of the deceased
- marriage or divorce certificate, if relevant
There is no charge for this procedure.

Please note that the original death certificate, including the apostille and translation, will be kept at the Special Registry Office in Brno. We therefore recommend that you apply for a duplicate at the British registry office.
The translation must always be done by a qualified (certified or court-appointed) translator, self-translations cannot be accepted. If you choose a certified translator, additional certification fee will be charged.
