passport application - adults (18+)
There are two conditions that need to be fulfilled for an application for a Czech passport – the applicant should have a valid proof of Czech citizenship (see below) and a proof of personal number “rodné číslo”, such as Czech or Czechoslovak birth certificate / marriage certificate (see paragraph 1.). Otherwise, the applicant shall obtain certificate of Czech citizenship and/or Czech birth certificate first before applying for the passport (see paragraph 2).
1. THE APPLICANT HAS A VALID PROOF OF CZECH CITIZENSHIP AND A PROOF OF PERSONAL NUMBER (= valid Czech ID card OR valid Czech passport OR valid certificate of Czech citizenship (issued in last 12 months) + Czech / Czechoslovak Birth Certificate).

- One of the above mentioned options of proof of Czech citizenship (and the Czech birth certificate, if applicable – see above)
- Czech marriage certificate – in case of a changed surname after a marriage
- Czech document approving of a change of name / surname – in case of a requested and approved change of name /surname;

- Application fee according to the current schedule of consular charges

NOTE: All forms must be printed out and thoroughly filled in and presented during the appointment.
2. THE APPLICANT DOES NOT HAVE ANY VALID PROOF OF CZECH CITIZENSHIP (= no valid Czech ID card / valid Czech passport / both: valid Certificate of Czech Citizenship (issued in last 12 months) + Czech / Czechoslovak Birth Certificate)
Please note that it is not possible to apply for a new Czech passport without a valid proof of Czech citizenship. Therefore, it is necessary to apply for certificate of Czech citizenship first. As soon as the applicant receives the certificate of citizenship, it is possible to apply for a new Czech passport. Please note that birth certificate, marriage certificate and driver licence are not considered as a proof of citizenship.
Application for the certificate of the Czech citizenship - mandatory documents:
- Czech or Czechoslovak birth certificate – if the applicant does not have the birth certificate available, he / she can apply for a its duplicate along with the certificate of citizenship;
- Proof of identity – previous Czech passport or identity card / foreign passport/ driving licence etc. with an up-to-date photo. If the applicant does not hold any relevant proof of identity (see above), then:
- A witness with his / her valid ID card or passport who will vouch for the applicant shall accompany the applicant at appointment too.

- Further relevant documents: marriage certificate, death certificate of the previous husband / wife etc. Please note, that all marriages, deaths and births that occur outside the Czech Republic must be registered with the respective Czech institutions.
- Previous certificate of Czech citizenship, if available.
- If applicable: proof of another citizenship (i. e. certificate of naturalization) with an apostille / superlegalization, fully translated in Czech by a court appointed (sworn) translator. If the proof of citizenship (certificate of naturalization) is issued by an EU country, a simple translation in Czech language done by a certified translator is sufficient and the apostille is not required.
