An emergency passport is issued to Czech citizens for an immediate one-way journey to the Czech Republic, and only in certain cases of proven emergencies.
In urgent emergency situations where the Czech national needs to travel to the Czech Republic for serious reasons and does not have a valid (Czech) passport, for example, due to theft or sudden loss, we can issue an emergency travel document, which is is issued for a very limited period of time, covering a single one-way journey to the Czech Republic only.
required documents for applicants aged 0-17:
- printed and completed forms:
- Personal data (parent or child 15+)
- Declaration (child 15+) or Declaration (child 0-14)
- Declaration (parent)
- Application
- original of the child’s Czech birth certificate or foreign birth certificate with apostille and translation into Czech by a court-appointed translator
- valid passport/national ID of the parent/holder of parental responsibility
- any previous passports/national IDs or Certificates of Czech citizenship of the child
- proof of the child acquiring foreign citizenship with an apostille and translation into Czech by a court-appointed translator, if relevant
- two passport-sized photographs of the child
- tickets for a flight within 10 days of the appointment
The parent/holder of parental responsibility is required to attend the appointment in person together with a child aged 6+ to submit the application.

- printed and completed forms:
- a photo identification document (e.g., previous passport or national ID card; driver's license) from which your identity can be clearly determined. If you do not have such a document, the presence of a witness with a valid passport or national ID card who can confirm your identity is required.
- all other relevant documents that help establish your identity (photo ID cards, birth or marriage certificate, etc.)
- two photographs in the format 3.5 x 4.5 cm
- flight tickets for a flight within 10 days of the appointment
In case of loss or theft of travel documents in the United Kingdom, we recommend reporting the incident immediately at the nearest police station, where you should receive a report of the loss (so-called Property lost in streets) or theft. Subsequently, we recommend reporting the loss in person at our office, where we will issue a confirmation of the reported loss/theft and notify the relevant municipal authority.
Proof of emergency situation is required before an appointment is made. An emergency passport is not issued as a ‘replacement passport’ in case of a national residing in the UK, just wishing to go on holiday. Appication for a stadard passport must be made in such a case.

The emergency passport is issued for a very limited period of time, covering a single one-way journey to the Czech Republic only. The validity of the document is usually 10 days, during which the holder must get to the Czech Republic.
Once in the Czech Republic, the emergency passport must be returned to the passport office.
