CERTIFICATE OF CZECH CITIZENSHIP (15+)
Persons of 15 and more years of age are applicants entitled to submit their application for the Certificate of Czech Citizenship and thus, must attend the appointment in person.
I HAVE CZECH BIRTH CERTIFIFACE (OR I HAD IT IN THE PAST)
WHAT DO I NEED?
- ID with a photo - a Czech passport/an ID card or a driving license/a British passport
- If you do not have a valid ID, you need to come with a witness who has a valid ID (a passport/an ID card) who will confirm your identity to us, and present your passport-sized photo.
- A Czech birth certificate - if you do not have it, you can apply for a duplicate during the appointment – additional fees may apply.
- Proof of marital status, if not single - e. g. Czech marriage/divorce/death certificate*
- Evidence of acquiring other citizenship*, if applicable
- The application fee (in accordance with current consular rates).
- Printed and filled out forms:
*British documents must bear an apostille and must be translated into Czech by a sworn/court-appointed translator. Court-appointed translators, whose translations do not need any further verifications, are highlighted.
If you are submitting a document issued in another country, check the verification and translation requirements for the specific country.
The applicant who provides false information is a possible subject to prosecution.

WHAT DO I NEED?
- A valid ID with a photo (passport/national ID card or a driving license)
- If you do not have a valid ID with a photo, you need to come with a witness who has a valid ID (a passport/an ID card) who will confirm your identity to us, and present your passport-sized photo.
- Birth certificate*
- Proof of marital status, if not single - e. g. a Czech marriage/divorce/death certificate*
- Evidence of acquiring other citizenship*
- Birth certificates of your parents*
- A marriage certificate of your parents, if they were married*
- Death certificates of your parents, if they are deceased*
- Certificate of Czech Citizenship of your parents, if available
- Further supporting documents/evidence related to your Czech citizenship* (e.g. evidence of adoption, if you have been adopted)*
- The application fee (in accordance with current consular rates).
