notarization of signature

Notarization of signature on a document to be used in the Czech Republic may be done by our Consulate or by any Honorary Consulate within our jurisdiction.

You must come in person and sign the document in presence of the consular officer during the opening hours for public, preferably early just after opening but the latest 30 mins before closing. A prior appointment is necessary. Include the number of requested notarizations in your query.

Requirements:

1. document to be signed (it is not possible to print any documents in the Consulate)
2. valid proof of identity - valid Czech passport, valid Czech national identity card (občanský průkaz), valid U.S. passport or valid U.S. driver's license
3. consular fee in cash in USD (item 150a)

NOTE:
Notarizations done by the Czech Embassy or Consulate are only valid for use in the Czech Republic, not in the U.S. If you need the notarization to be valid in the U.S., it must be done by an U.S. notary public or by the U.S. Embassy or Consulate.

U.S.apostille

If you need to certify the authenticity of a U.S. document (eg U.S. birth, marriage or death certificate) to be used in the Czech Republic, the document must be apostilled by the competent U.S. Secretary of State according to the place where the document has been issued. The list of competent authorities can be found on the website of the Hague Conference on Private International Law.

U.S. document without the Apostille will be not accepted by the Czech authorities.
Czech Consulate cannot issue U.S. Apostille.
