Please schedule an appointment at toronto.appointment(at)mzv.cz and come in person at the confirmed time and date to the consulate with your passport and temporary protection as well as with a twice filled-out form, which we will send you.  In the case of minor children, it is sufficient for the legal guardian to be present.

The consulate does invalidate visa with temporary protection in the  travel document but does not make any records; the consulate verifies the signature and submits the form to the Czech Republic.
