Applications in the  registry and citizenship matters can be submitted within the framework of consular days. These are mainly the following applications:

• - Czech citizenship certificate,
• - acquiring the Czech citizenship by declaration,
• - registration of birth, marriage, death (i.g. issuance of Czech birth, marriage, death certificate),
• - duplicate od registry document
• - verifications (signature, translation, copy)

as well as to apply for a registration to the special list of voters of the Czech Embassy in Ottawa. Please note that this registration is necessary in order to be able to vote by correspondence.

To submit the application, you need to have an appointment. 

For the appointment, please call +1 613 562 3875 ext. 11.

During a phone call, you will be asked to provide:

• - name and surname,
• - phone number,
• - email and
• - type of the application you wish to submit,
• - applicant, wishing to submit also applications for his/her children will also need to provide their names and surnames as well as their age.

Please have this information ready for your call.

After setting up the appointment by phone, you will receive its confirmation by email.

The consular fees will be collected in CAD, cash only. 

The applicants with the appointment will wait at the reception.
