Czech Citizenship Certificate

Information on how to obtain a formal proof of Czech citizenship.

Purpose

Certificate of Czech citizenship (CCC) is one of the formal proofs of Czech citizenship (the other being valid passport, valid "občanský průkaz" and a statement of non-impediment to marriage). The certificate of Czech citizenship must be presented with  a passport application if the applicant's previous Czech passport expired and he/she has no other proof of citizenship (exceptions may apply).  A certificate of Czech citizenship is issued with the validity of 1 year. A birth certificate of the Czech Republic does not prove a citizenship.

How to submit the application?

In person - you need to make an appointment for a personal submission by sending the request to the email address: Ottawa.Consulate@mzv.gov.cz

We would also appreciate receiving a pre-paid trackable envelope with your return address. We recommend using Xpresspost envelopes.

In  case of submitting an application for the issuance of an CCC regarding a person younger than 18 years of age, his/her parent submits the application on his/her behalf and signs the application.

The consular staff will help you to clarify any questions, but they cannot fill out the forms for applicants.

Honorary consulates are not entitled to deal with registry and citizenship issues - in exceptional cases, due to considerable geographical distances, honorary consul can verify the signatures of the applicant on applications forms. Honorary consul  is not responsible for the correctness of submitted applications and it is therefore necessary for applicants to contact the Embassy of the Czech Republic in Ottawa with all questions they may have regarding their application.

All applicant's signatures must be verified by the honorary consul of the Czech Republic in Canada on the forms sent by post.

All copies of the documents attached to the application must also be verified, unless you are also sending their originals. Copies of documents can be verified either by the honorary consulates of the Czech Republic in Canada (then no further verification is required) or by a Canadian notary, in which case it is also necessary to obtain the apostille on the document;  Copies notarized only by a Canadian notary public will not be accepted.

When sending an application by post:

- one of the forms of payment of the consular fee must be enclosed in an envelope for the Embassy of the Czech Republic in Ottawa

- a pre-paid Xpresspost envelope. If the prepaid envelope is not sent together with the application, the applicant must pick up the issued document in person at the consular section (after arranging an appointment at Ottawa.Consulate@mzv.gov.cz, or the document will be sent back by the regular mail).

If you intend to apply for Czech travel passport, you can pick-up your Czech Citizenship Certificate when applying for the new passport.

How to apply


A. Applicants born in the Czech Republic (Czechoslovakia):

  1. Application form (PDF, 181 KB) and Questionnaire (PDF, 190 KB) - these  forms are to be completed in th entirety, incl. the questions that are not applicable (see the explanation note at the end of the form); the applicant is always the person for whom the certificate is to be issued, irrespective of his age (legal representatives sign the forms for minors);
  2. Czech (Czechoslovakian) birth certificate; Marriage certificate (if applicable), with necessary verifications (apostille, if Canadian) and translation
  3. Note: If your current last name is different than the one that appears on your birth certificate, the document leading to such change must be presented. If you do not have it, you can at the sam time submit the application for the Czech marriage certificate;
  4. Death certificate or divorce certificate, if applicable.
  5. Proof of acquisition of foreign citizenship containing the date of such acquisition, with necessary verifications (apostille, if Canadian) and translation;
    If you acquired Canadian citizenship, certificate of Canadian citizenship is required in the form of the Proof of Oath (the diploma-like document). If you do not have it, contact Citizenship and Immigration Canada to obtain one.
    Applicants who have not acquired Canadian citizenship must prove this by presenting a "permanent resident card" or a passport with a valid Canadian visa or any other document proving that they are not Canadian citizens;
  6. Personal Information form (DOC, 24 KB) ;
  7. Valid ID - when sending your application by mail, attach the copy of the document which you used as your ID when verifying your signature on the forms at the Czech honorary consulate;
  8. Consular fee for the Czech Citizenship Certificate (please note that several consular fees might be connected with your application, depending on the set of your documents, such as for example the fee for the verification of the copy, for the verification of the translation).

 

B. Applicants born in Canada who had never been issued any Czech document proving citizenship and Czech birth certificate

  1. Application form (PDF, 181 KB)  and  Questionnaire (PDF, 190 KB) - these forms are to be completed in its entirety, incl. the questions that are not applicable (see the explanation note at the end of the form); an applicant is always the person for whom the certificate is to be issued, irrespective of his age (legal representatives sign the forms for minors);
  2. Birth certificate of the Czech parent, if both parents are Czech, birth certificate of each of them; documents on the Czech citizenship of parent(s), if available;
  3. Czech marriage certificate of applicant's parents (if married at the time of the applicant's birth) - if the parents only have a Canadian marriage certificate, it must be submitted in original, with the apostille, and translated into Czech. The application for the Czech marriage certificate has to be submitted at the same time;
  4. Canadian birth certificate, in the long form, it must be submitted in original, with the apostille and translated into Czech. It is recommended to apply for the Czech birth certificate at the same time.
  5. Personal Information form (DOC, 24 KB);
  6. Valid ID - when sending your application by mail, attach the copy of the document which you used as your ID when verifying your signature on the forms at the Czech honorary consulate;
  7. Consular fee for the Czech Citizenship Certificate (please note that several consular fees might be connected with your application, depending on the set of your documents, such as for example the fee for the verification of the copy, for the verification of the translation).

C. Applicants born in Canada who had been previously issued Czech document proving citizenship and Czech birth certificate

  1. Application form (PDF, 181 KB) and Questionnaire (PDF, 190 KB) - these  forms are to be completed in th entirety, incl. the questions that are not applicable (see the explanation note at the end of the form); the applicant is always the person for whom the certificate is to be issued, irrespective of his age (legal representatives sign the forms for minors);

Additional info

The processing time is 2-3 months in case of applicants in the groups A, 2-4 months for applicants in group B and C.
