CITIZENSHIP CERTIFICATE

PURPOSE OF THE DOCUMENT
Citizenship certificate issued in last 12 months is a proof of Czech citizenship. Its possession is not mandatory as a valid passport or national identity card ("občanský průkaz") can be used as a proof of Czech citizenship. However, it is necessary to obtain and submit citizenship certificate when a person wishes to apply for a civil registry documents or a Czech passport abroad and does not possess other proof of citizenship issued in the Czech Republic (valid passport, občanský průkaz, or pre-marriage approval containing the citizenship status).
You can submit a request for a certificate of citizenship at our consulate.
Consular District of the Consulate General in New York
Please note that an application can only be submitted to the Consulate General in New York if the applicant resides in one of the states within the consular district: Connecticut, Delaware, Maine, Massachusetts, New Hampshire, New Jersey, New York, Pennsylvania, Rhode Island, Vermont. Residency in the consular district can be proven by a copy of an ID with the listed address or, for example, a utility bill from the place of residence.
The application is sent to the Czech Republic for processing. The relevant regional authority decides on the application. The Consulate General does not make decisions on the application or on the documents that need to be submitted with the application. The Consulate General only serves as an intermediary for applications, documents, and paperwork between the applicant and the relevant authority.
APPLICATION
1. Complete the following forms:
How to fill out the form - a practical guide:
- * You have to fill in only the Czech version of forms!!!
- *All pages must be filled out legibly.
- *The application must include the date, place of signing, and your signature at the end.
- *If you sign the forms outside of the consulate, your signature must be notarized (a Notary Public in the USA is sufficient).
- *Information about your spouse must be filled out even in the case of divorce or death.
- *The section of the questionnaire titled "Prohlášení o rodičích" must always be completed, both for children and adult applicants.
Filling out forms for children (under 18 years old):
In the "Žádost" form, the legal guardian is listed as the applicant on the first page.
In the "Dotazník" form:
- *On the first page, enter the child's information, and on the second page, enter the legal guardian's information.
- *The second page must always be signed by the legal guardian.
- *The third page is filled out and signed by the legal guardian for children under 15 years old. Children aged 15-18 do not need to submit this page.
- *The fourth page, "Declaration of Parents," is signed by the legal guardian for children aged 0-15. Children aged 15-18 sign this page themselves.

2. Valid Identity Document
If you do not have any Czech identity documents, the document can be replaced by a foreign identity document (passport, driver's license from another country, or ID). A valid U.S. Green Card can also be presented as an identity document.
If you are not submitting the application in person at the consulate, you must send the original identity documents or their notarized copies.
3. Civil Registration Documents to Be Attached to the Application
a) Birth certificate,
b) Marriage certificate, certificate of partnership, divorce decree, dissolution of partnership, death certificate of a deceased spouse or partner – if applicable,
c) Birth certificates of parents, their marriage certificate, divorce decree, their death certificates if they are deceased, if the applicant has these documents,
d) Previously issued certificates of the applicant's citizenship and the citizenship of the parents, or possibly other ancestors, if the applicant has these documents,
e) Adoption certificate, if the person for whom the certificate is requested was adopted by a Czech citizen,
f) Proof of acquisition of foreign citizenship containing the date of such acquisition. A document proving the date and reason of acquiring foreign citizenship (e.g., U.S. naturalization certificate – if the applicant does not have a naturalization certificate, they must request a duplicate from the local office of the Bureau of Citizenship and Immigration Services. The naturalization certificate can be submitted without an apostille and without an official translation into Czech).
Documents concerning the applicant directly (point a) issued by foreign authorities must be accompanied by the necessary certifications (e.g., an apostille for documents issued in the USA) and an official translation into Czech, except for documents issued in Slovak.
SUBMITTING THE APPLICATION
1. By mail
If you are submitting the application by mail, the signatures on the forms and copies of the submitted documents must be notarized. A copy of the U.S. naturalization certificate does not need to be notarized.
We highly recommend including a cover letter with basic information – address, contact details, what you are requesting from the consulate, whether additional applications for civil registration events or a passport will follow the certificate request, and a list of documents attached to the letter, etc.
If you would like us to return the original documents to you (and you will not be picking them up during a future visit to the Consulate), you must include a prepaid return envelope.
2. In person
You can submit the application in person at the Consulate by appointment during office hours. Please schedule an appointment via email at newyork.consulate@mzv.gov.cz.
Please note that if the applicant does not complete the forms before the appointment, the application cannot be accepted, and another appointment will need to be scheduled.
When submitting in person, notarized copies and signatures are not required, as the Consulate will confirm that they have seen the original documents and will automatically verify your signature, which you will provide in the presence of the consular officer.
Appointments are scheduled in the order in which emails requesting appointments are received.
FEE

The fee can be paid by:
- *Cash
- *Money order (payable to "The Consulate General of the Czech Republic in New York, 321 East 73rd Street, New York, NY 10021")
- *Credit card during an in-person visit, except for American Express cards.
PROCESSING TIME
Once the completed application and all documents are submitted, our office forwards them to the relevant authorities in the Czech Republic. Processing takes approximately 2-3 months from the time the complete application is sent to the Czech Republic. In cases where archival research is required, the processing time may be longer.
Once the certificate is issued, the applicant will be contacted by email. The certificate can be sent by mail (the applicant must provide a prepaid envelope) or picked up in person when arranging for other documents or a Czech passport (an appointment must be scheduled in advance).
