We also continue to encourage applicants to submit eligible applications by mail whenever possible. This option saves time otherwise spent arranging an in-person appointment, taking time away from work or other commitments, and traveling to our office.
If your application and all supporting documents are properly prepared, you only need to have the signatures on the application and accompanying documents notarized by a U.S. Notary Public and mail the complete package to our office. Upon receipt, we will review the submission and, if it is complete and without deficiencies, we will send you an email with a link to pay the applicable consular fee through our online payment system.
To obtain a duplicate of a Czech Birth/Marriage or Death Certificate please send the following:
1.
filled out the application form, your signature on the application form must be verified by notary public
2.
copy of your valid passport or valid driver's license or valid state ID
3.
in case you are relative, you need to provide original civil registry documents proving it, in case you are not relative, enclose explanation letter why do you need the birth /marriage or death certificate
4.

If you wish to apply for the Apostille, you need to include additional fee which is under number 150e

filled out and signed Statement form
If you would like us to send your Czech document to your address, please send us a pre-paid USPS Priority Express Mail envelope - EP13F (only overnight express mail with a tracking number).
For future tracking, we suggest you to make a record of the tracking number.
