Applications for an extract from the Criminal Register of the Czech Republic may be submitted at the Embassy of the Czech Republic in Tbilisi.
The processing time is approximately one hour. However, if an apostille is also required, the processing time is extended to approximately two months.
Required Documents
Applicants are required to submit the following:
A duly completed application form.
A valid photographic identity document (national identity card or passport).
Payment of the applicable consular administrative fee (see the Schedule of Consular Fees).
Applications for an extract from the Criminal Register cannot be submitted by post.
Verification of Identity and Personal Data
When applying for an extract from the Criminal Register, the applicant’s identity and the accuracy of the personal data provided in the application must be verified.
Verification of Identity
The applicant’s identity is verified based on an original, valid photographic identity document (national identity card, passport or residence permit).
If an application is submitted by a person acting on behalf of the applicant, that person must also present an officially certified power of attorney.
Verification of Personal Data
The accuracy of the information provided in the application is verified against an original, valid identity document (e.g. a national identity card) containing at least the following information: first name, current surname, surname at birth, date and place of birth, personal identification number and citizenship.
If any of these details are not included in the identity document presented (e.g. passport, residence permit or certificate of loss of identity card), the applicant or their authorized representative must provide additional supporting documents, such as a birth certificate, baptismal certificate, certificate of birth or marriage certificate.
Citizens of the Czech Republic and the Slovak Republic must submit the originals or officially certified copies of these documents.
Citizens of other countries must submit the originals or officially certified copies accompanied by an official translation into the Czech language.
This requirement arises from the fact that the records of the Criminal Register are maintained based on individuals’ surnames at birth.
Submission of Applications by an Authorized Representative
Applicants who require an extract from the Criminal Register of the Czech Republic but are unable to submit their applications in person may authorize another person to submit the application and collect the extract on their behalf.
Issuance of Multilingual Standard Forms
As of 29 September 2026, the Ministry of Justice of the Czech Republic has implemented a technical measure in accordance with applicable European Union legislation, restricting the issuance of multilingual standard forms accompanying extracts from the Criminal Register exclusively to citizens of the Czech Republic.
