The requrirements for the application are:

• a properly filled application form
• a valid ID (passport, ID card etc.)
• fee

If you apply for a Czech criminal record for use in Taiwan, do not forget to also apply for a Czech superlegalization if such procedure is required by Taiwanese authorities. The Office will forward the application to the competent authority in the Czech Republic. Currently the process takes approximately 2-3 months.

If the application is not submitted in person, the signature on it must be verified by a notary public and identification documents must be submitted in the form of an authenticated copy (a bank authentication is sufficient). If you apply on behalf of a third person, please enclose the power of attorney verified by a notary public with its Czech translation.
