Loss/theft of Czech residence card
If you are in Singapore and have lost your Czech residence card ("průkaz o povolení k pobytu") issued by the Ministry of the Interior of the Czech Republic, you have a duty to inform the Embassy in maximum 3 days.
You need to schedule an appointment in advance by e-mail.
Please fill out this Residence card loss/theft form (PDF, 97 KB) and bring it together with the police report of loss or theft. If you have it, please bring a copy of the lost residence card.
