Duplicate of a Czech Birth, Marriage and Death Certificate
Czech citizens are obliged to register births, marriages and deaths that occurred outside the Czech Republic with the Office of Vital Records in Brno, Czech Republic. The life events that occured within the territory of the Czech Republic are registered in the respective municipality's registry. Therefore, if a registration was already done in the registry in Brno or elsewhere, it is possible to apply for a duplicate of the document.
Important information: When submitting the application, it is necessary to know in which municipal registry in the Czech Republic the person's life event is registered in!
To proceed with the application for a duplicate you need to bring:
- completed form: žádost o duplikát matričního dokladu (PDF, 351 KB)
- passport or photo ID
- in case you are not the person mentioned in the certificate, enclose an explanation letter why do you need the duplicate
- the fee for issuance of the duplicate of the birth certificate
Consular administrative fees can be paid at the Embassy's Consular Section either in cash in Singapore Dollars (SGD) or by bank card (VISA, Maestro, Mastercard, DinersClub, Discover; American Express cards are not accepted).
