Czech Criminal History Record
The Czech Criminal History Record is issued by the criminal registry authority "Rejstrik trestu" in the Czech Republic. For those who currently reside in Singapore, it is possible to apply for it at the Embassy of the Czech Republic in Singapore.
Applicants need to schedule an appointment in advance by e-mail ONLY! The address and office hours of the consulate can be found in the "about the Embassy" section of our website.
To obtain the Criminal History Record you need to:
1) complete the Application form (PDF, 230 KB):
2) Bring your valid passport.
3) Pay the fee.
If you need to affix Apostille on the document, please inform the consular officer when submitting the application.
