To apply for the criminal record certificate, complete the following form: Complete form (PDF, 220 KB) and bring an identity document (passport).

To make an appointment, please contact us at mumbai.consulate@mzv.gov.cz.

The application is subject to an administration fee.

The processing time is approximately 8 weeks.

- If applying instead of the person for whom the certificate was issued, present the applicant´s power of attorney (DOCX, 13 KB).

- For use abroad, additional authentication by the Ministry of Foreign Affairs of the Czech Republic may be required. If apostille is required, you can apply for it via the Consulate General in Mumbai during your appointment. Issuance of apostile is subject to fee.
