REQUIRED DOCUMENTS

• - original travel document (a travel document for the purposes of conferring a long-term visa shall not be older than 10 years, it must contain at least 2 free pages and its validity must be at least 3 months longer than the validity of the long-term visa)
• -  2 passport size photograph (showing current appearance) 
• - Qatari ID
• - application form (below the whole text in attached file)
• - accomodation
• - purpose of the stay - registry extract, such as trade licence extract (živnostenský list) or company register extract (výpis z obchodního rejstříku) or other
• - financial means
• - criminal record (police clearance certificate) issued by the state of which you are a citizen, as well as the states in which, in the last 3 years, you have resided continuously for a period longer than 6 months, or a signed affidavit in the event that this state does not issue such a document 
• - fee
• - biometrical data

 

BOOKING OF APPOINTMENT

Every applicant must apply for the appointment in advance via e-mail doha.consulate@mzv.gov.cz

 

DO NOT OVERLOOK

-Supporting documents may not be older than 180 days with the exception of a passport, registry documents (birth certificate or marriage certificate), diplomas and similar documents and photo (if reflecting current appearance).

-Supporting documents must be submitted in Czech. Documents in another language must also be submitted in official translation into Czech.

-Foreign public documents must be legalized, i.e. equipped by an apostille or superlegalized.

-The following documents must always be submitted in their originals: passport, supporting documents on purpose of stay (in this case employment permit). These originals will be returned to the applicant should he / she wish to keep them. Passport is always returned to the applicant and is submitted again when issuing the visa.

-Other supporting documents may be submitted as originals or officially verified copies.

If your application is approved, prior to affixing the visa, you are obliged to submit a proof of medical travel insurance that corresponds to the specified conditions (not required if the foreign national subscribes to public health insurance or if the costs for health care are covered on the basis of an international agreement or if the foreign national can demonstrate that health care is covered in another manner), and, upon request, a document on paying the insurance presented in the proof of medical travel insurance

-SUBMIT ONLY COMPLETE APPLICATIONS CONTAINING ALL THE DOCUMENTS.

-INCOMPLETE APPLICATIONS ARE UNACCEPTABLE => THEY WILL NOT BE ACCEPTED FOR PROCESSING AND WILL BE RETURNED TO THE APPLICANT. (§ 169h of Act on Residence of Foreign Nationals)

- The legal processing time of long-term visas is 90 days. The legal processing time can be extended to 120 days in individual cases requiring more examination.
