Registered partnership

Registered partnerships of citizens of the Czech Republic (at least one of the partners) that took place on the territory of a foreign state are registered in the Special Registry Office in Brno.
What needs to be submitted:
1) Completed and signed Application Form
- Application for registration of a registered partnership
- All requested information is mandatory, filled in CAPITAL LETTERS.
- The dates should be written according to European standards; i.e. DD/MM/YYYY
- The name of the city or town and the country should be given as the place of birth; it is not sufficient to give only the country of birth.
- If a person has a middle name, this must be provided; only the initial letter is unacceptable.
- No abbreviations may be used.
2) Original foreign-issued document of the registered partnership
- The document must be apostilled and officially translated into Czech. The list of translators in Australia and New Zealand can be used if necessary.
3) Original birth certificates of both partners
- A citizen of a foreign country must submit only the original foreign birth certificate (if they do not have a Czech birth certificate) with an apostille and official translation.
4) Original proof of Czech citizenship and/or proof of identity
- The Czech citizen must present: a valid passport of the Czech Republic or a valid ID card of the Czech Republic, in case of invalidity of these documents, a Certificate of citizenship of the Czech Republic not older than one year from the date of issue
- A foreign citizen must present a valid passport or other document proving identity and nationality.
5) Administrative fee
- A fee is charged for the application for a registered partnership and the Embassy of the Czech Republic in Canberra accepts only cash payments.

7) Original proof of dissolution of marriage/registered partnership
If you entered into a registered partnership as a divorcee:
- If the marriage was dissolved outside the Czech Republic, you need to register the foreign divorce decision with the Supreme Court of the Czech Republic based in Brno before registering your marriage. The Embassy of the Czech Republic in Canberra cannot process these applications and you must apply directly to the Supreme Court of the Czech Republic based in Brno.
- The Czech citizen must provide a divorce decision recognised by a Czech court (the Supreme Court of the Czech Republic based in Brno - if the divorce took place abroad, or the court of the place of divorce in the Czech Republic).
- A citizen of a foreign country may submit a foreign divorce decision which is apostilled and officially translated
8) Death certificate of the deceased spouse or partner
If one of the partners entered the marriage as a widower
- A citizen of a foreign country only needs to submit the original foreign birth certificate, which is apostilled and officially translated
9) Application for registration of a female surname in the male form
If you wish to give your female surname in the male form, i.e. without the ending "- OVÁ".
- In the case of the first entry, this is the form "First entry of a female surname in the male form"
- In the case of a previous entry, this is the "Additional entry of a female surname in the male form"
