Request for appointment only via e-mail: konsulat_berlin@mzv.cz

The email must contain the following identification information about the applicant:

• full name, date of birth, number of the passport

• country that issued your passport (nationality of the applicant)

• scan of the proof on the purpose of stay (e.g. study, employee card, scientific research, business, family unification etc.)

• scan of your German/Danish residency permit 

• period of your intended stay in the Czech Republic

The applicant will receive an appointment only if the e-mail contains all necessary requirements.
