• Submit your proposal with unit price(s) in CZK / EUR, as instructed, on or before closing time;
• Note that failure to submit proposal in CZK / EUR, or in accordance with the requested terms, will result in automatic invalidation of your application;       
• Submit all documents in English, using the enclosed format. Local Small Scale Project Identification Form;
• Budget must be between 200.000 – 500.000 CZK and total sum of budget lines 2 and 3 cannot exceed 25% of the overall budget.  

SUBMISSION TERMS:

In order to be considered eligible, your Local Small Scale Project Identification Form proposal should be hand delivered:

• In sealed envelope to the Czech Embassy, Chavchavadze Avenue 37-block VI, 0179 Tbilisi on or before November 15^th 12:00hrs, 2015;
• To email tbilisi@embassy.mzv.cz in Excel format.  

The envelope must contain:

• Properly filled Local Small Scale Project Identification Form;
• Notary certified extract from the Register of Legal Entities in English.
