Surrendering temporary protection granted in the Czech Republic via the Consulate General of the Czech Republic in Sydney

Ukrainian citizens who express interest in surrendering temporary protection issued in the Czech Republic may contact the Consulate General of the Czech Republic in Sydney under the following conditions:
1/ The applicant and holder of temporary protection must attend the Consulate General in person (should the applicant be an underage child, the presence of a legal representative or guardian is sufficient). The applicant must request an appointment via email. The Consulate General will verify the applicant's identity based on their travel document and temporary protection.
It is also possible to surrender temporary protection in the Czech Republic without coming to the Consulate in person. In this case, the applicant’s signature on the form must be officially verified. The completed form must be addressed to the Consulate General of the Czech Republic and sent via post (NOT via email). Signature verification can be performed by an Honorary Consulate of the Czech Republic. Should the signature on the document be verified by a local JP or notary public, it is necessary to obtain a higher form of verification (Apostille) in addition.
2/ The applicant should complete the form twice.
3/ The Consulate General will hand over one version of the form to the applicant during their appointment. The second original will be sent to the Ministry of the Interior of the Czech Republic. Should the applicant have lodged their applicant via post, one original will be kept at the Consulate General for collection. Originals will not be sent back to the applicant via post.
The Consulate General accepts applications to surrender temporary protection from all Ukrainian citizens, regardless of their address i.e. regardless of whether or not they fall in the relevant consular district.
Health insurance – applicants must fulfil their responsibility to inform the necessary health insurance provider with whom they registered with in the Czech Republic. For those insured by VZP, this can be done by sending a scan of the document “Prohlášení o vzdání se oprávnění k pobytu za účelem dočasné ochrany” (Confirmation of the surrender of temporary protection) to the following email: info@vzp.cz. Health insurance ends on the date that temporary protection is annulled.
