Schengen visa for the purpose of tourism

Visa requirements
 

1. Schengen visa application form (duly and completely filled in using roman characters and signed by the applicant; in the case of persons under 15 years of age, signed by a parent or a legal guardian)

2. passport or other travel document (original + copy of the data page and pages with remarks and pages with previous Schengen visas; the document must not be older than 10 years, it must contain at least 2 blank pages and its validity must extend beyond the validity of the visa to be granted by at least 3 months)

3. permanent or long-term residence permit in Canada, valid for at least 3 months beyond the validity of the visa to be granted  + copy

4. document certifying the applicant's residence within the consular jurisdiction of the Embassy of the Czech Republic in Canada + copy

5. 1 colour photograph (size 3.5 x 4.5 cm, white background, must match the applicant's actual current appearance, frames of glasses must not cover the eyes)

6. travel health insurance (valid for all EU/Schengen countries, minimum coverage of EUR 30 000, must cover at least the period of the first trip if applying for a multi-entry visa;

7. travel confirmation (original or copy)

                 - organised tourism (with a travel agency) - confirmation of the travel order from the host organisation (voucher), which must include the applicant's details, proof of advance payment or full payment, the stamp of the inviting organisation and the signature of the responsible person

                 - individual tourism - accommodation reservation and detailed itinerary

8. transport information – flight ticket booking

9. proof of employment

10. proof of sufficient funds to cover the costs of the trip

               - the applicant's bank statement, or

              - sponsorship - notarised declaration of sponsorship and proof of sufficient funds from the sponsoring person

11. visa fee

Children under 18 year of age travelling unaccompanied or with only one parent

             - written consent of one/both parents (when not travelling with the child) - this document must be notarised and submitted in original and copy; if there is only one legal guardian, then a birth certificate or a court order of sole custody or a death certificate of the other parent or another document explaining the situation, is required

             - a copy of the parent's travel document

             - birth certificate - original and copy
