Czech passport
Passport application can only be submitted in person at the embassy (honorary consulates do not accept passport applications).
To apply for a passport, you need to make an appointment at the email address Ottawa.Consulate@mzv.gov.cz.

Before submitting your request for the appointment, please verify that you belong to the consular jurisdiction of our embassy.
The passport is issued by the Interior Ministry (Ministerstvo vnitra), remains property of the Czech Republic and can be withdrawn at any time. It serves as a prove of Citizenship according to the Czech nationality law. If substantiated, citizens can hold multiple passports at the same time. Children can no longer be included in the passport of their parents.
The biometric passports have been issued since 2006 with a ten-year validity, and for citizens under fifteen years of age with a five year validity.
Applicants must submit the following:
Previously issued Czech/Czechoslovak passport, if applicable;
Document proving identity - permanent residence card, travel passport, driving licence, etc.
Document proving Czech citizenship - e.g. previously issued but still valid Czech passport, valid "občanský průkaz" (national ID card), certificate of Czech citizenship (must not be older than 1 year);
Document containing "rodné číslo" (unique personal number) - either birth certificate (if it contains rodné číslo), passport, Czech ID (občanský průkaz) or other official document issued by the Ministry of Interior of the CR containing "rodné číslo";

The Procedure:
The applicant residing in the consular jurisdiction of the Czech Embassy in Ottawa emails a request for the appointment accomapnied by a filled and signed form for the verification of his/her data, mentioned above.
Once the embassy confirms the correctness of the entry in the Civil Register, the applicant receives via e-mail the appointment for submission of his application.
The applicants visits the embassy on the date and time of his appointment in person and submits all the required documents. The consular officer will scan the photograph of the applicant, scan their fingerprints and forward the application for further processing to the Czech Republic.
The passport will be manufactured in the CR and forwarded to the embassy. The applicant will be notified by email and will have to personally collect the document at the embassy.
To pick up the passport at the honorary consulate
The applicant may ask for the possibility to pick up the new passport at one of the honorary consulates of the CR in Canada, but he needs to express his wish to do so when submitting the application at the Embassy. It is also necessary that he provides two prepaid trackable mail envelopes for that purpose.
Processing Times:
The legal processing time is 120 days when applied from abroad, 30 days when applied in the Czech Republic. The average waiting time is two months when applied from abroad, 14 days when applied in the Czech Republic.
