Please note that applicants must first report to the building's lobby upon entering the building and request access to the Stikeman reception on the 41st floor. The reception will then direct applicants to the meeting room where the consular days will take place.

For technical and time reasons, applicants are required to book an appointment in advance:

by sending their application and appointment request to the following email address: Ottawa.Consulate@mzv.gov.cz

or by calling +1 613 562 3875 (ext. 11).

For this purpose, it will be necessary to provide the following information:

(1) first and last name,

(2) contact telephone number and e-mail address,

(3) description and number of consular services required (number of persons).

During consular days, applicants will be able to obtain following services:

passports – only the delivery of completed passports to applicants who have made an appointment; new applications cannot be accepted;

citizenship agenda – acceptance of complete applications for Czech citizenship certificates or restoration of citizenship;

registry agenda – applications for Czech birth, marriage, and death certificates (does not apply to recognition of divorce abroad);

verifications – verification of signatures, confirmation of life for pension payment purposes, certified copies, certification of the accuracy of translations;

CzechPoint agenda – collection of applications for criminal record extracts;

voting agenda – application for registration/removal from the special voter list kept at the Czech Embassy in Ottawa.

Consular fees associated with applications for specific consular services can be paid in CASH only. Please familiarize yourself with the specific fees for individual consular services, which must be paid on the spot in the exact amount.

Please note: Bring a valid ID (e.g., passport, driver's license) and a copy of it. It is also necessary to bring copies of the original documents that will be translated for further procedures (e.g., certification).

We recommend that you book an appointment as soon as possible, as consular services can only be provided to applicants with appointments. Thank you for your understanding.

Please note that out of consideration for other clients and due to technical limitations, incomplete applications will not be accepted, nor can legal or other advice be provided due to time constraints.

Please also note that the consular officer is able to help applicants on the spot to correct minor deficiencies in their applications, but due to time constraints, this assistance cannot be considered as comprehensive completion or translation of forms. For consultations that can be arranged with the consular section of the Embassy in Ottawa by phone or email, please use these forms of communication. Before your appointment, we recommend that you check the requirements for your application on our website.

We would like to thank Ms. Claire Zikovsky, Honorary Consul of the Czech Republic in Montreal, for providing the space.
