To apply for the criminal record certificate please submit the following documents

- Complete form (PDF, 220 KB)
- original of proof of your identity (passport).

At the same time you need to pay for the administration fee.

In case someone else instead of the applicant submits the application, the person has to present applicant´s Power of attorney (DOCX, 13 KB).

For submission of the application it is necessary to arrange a prior appointment via e-mail -  consular_delhi@mzv.cz

The processing time approximately takes 8 weeks.

For making use of the certificate abroad, an additional authentication by the Ministry of Foreign Affairs of the Czech Republic is necessary.
