Every person that needs her/his signature to be verified, must come in person to our Embassy with valid passport or ID, where is mentioned the whole name, date of birth, nationality and place of birth. This person must provide us with the address of residency (street, number of house, post code, city).

The appointment at our Embassy must be confirmed in advance by e-mail: doha.consulate@mzv.gov.cz
