Issuing a duplicate registry document

A duplicate of a civil registry document is issued if you have already been issued a civil registry document in the past, but now no longer have access to it (e.g. if you lose your birth, marriage or death certificate). You can apply for a duplicate registry document at the Embassy of the Czech Republic in Canberra, but also at the competent office in the Czech Republic (i.e. the municipal office in charge of the registry office).
Mandatory documents for the issue of a duplicate Czech civil registry document:
- Completed form "Application for a duplicate of a civil registry document", and
- Completed form "Consent to the processing of personal data"; and
- Proof of legal entitlement to the document in the case of a third-party applicant; and
- Valid ID (passport or Czech ID card).
When applying for more than one registry document, you must fill in a separate application for each document. The fee for issuing a duplicate of a Czech birth, marriage or death certificate is CZK 300. The current fee in Australian dollars can be found here (item 153a of the fee schedule).
To use a Czech government-issued document abroad, you must specify in your application whether you are requesting a higher verification of the document (Australia and New Zealand do require an Apostille). The fee for higher verification of the issued registry document (Apostille) is 300 CZK (item 150e).
If the application is made by post, all photocopies and signatures on the forms must be certified by the appropriate consular officer, Honorary Consul or Justice of the Peace (J.P. with official stamp and number) or Notary Public.
