How to proceed if you have an Australian death certificate and intend to apply for registration in the Czech registry office and for a Czech death certificate.
The most important supporting evidence for the registration of a death in the Czech civil registry that occurred in Australia is the Australian civil registry document (death certificate). The submitted Australian death certificate must first be certified with an apostille and then officially translated into Czech, including the apostille. A translator from the list maintained by our Embassy can be contacted to make the translation.

The following documents are required for the issuance of a Czech death certificate:
1. Application for registration of death; and
2. consent to the processing of the relative's personal data; and
3. proof of the deceased's Czech citizenship; and
4. apostilled original Australian death certificate and its official translation into Czech; and
if available:
5. birth and/or marriage certificate of the deceased; and
6. medical insurance card or a document showing with which medical insurance company in the Czech Republic the deceased was registered; and
7. proof of burial or cremation; and
8. a medical report - a certificate of examination of the deceased, if a medical or forensic autopsy was performed - a written notification from the doctor.
Note: An application for registration of the death of a citizen of the Czech Republic in the Special Registry Office in Brno is exempt from the fee.
If the deceased person did not hold a valid Czech ID document at the time of death, it is also necessary to apply for a certificate of Czech citizenship to prove that the deceased person was a Czech citizen at the time of death. An application for a certificate relating to the deceased person may be lodged by:
- a relative of the person in the direct line, sibling, widow or widower; or
- a natural person who demonstrates a legal entitlement to a certificate.
It is recommended that the application for Proof of citizenship be submitted at the same time as the application for registration of the death, with the proviso that the issued certificate of citizenship will be sent by the competent regional office to the special registry office in Brno.
