For Schengen visa applications, please submit the following supporting documents:

1. Documents confirming the purpose of travel

- for Business purposes/ conference / participation in sports events: Invitation letter from the host organization or organizer, or an official invitation form verified by the Foreign Police

- Visiting family or friends: Invitation letter or an official invitation form verified by the Foreign Police

- Medical purposes: Medical report from a Taiwanese healthcare or rehabilitation institution + Confirmation letter from a Czech medical facility specifying the type of treatment, duration of stay and total cost (including whether accommodation is provided) + Proof of prepayment or other proof of sufficient financial means

- Study purposes: Confirmation from the receiving institution in the Czech Republic (and possibly from the sending institution)

- Tourism in multiple Schengen countries: Travel itinerary explaining transportation between countries, Hotel reservations or other proof of accommodation for the entire stay

2. Letter from employer

- Must include: duration of employment, applicant’s position, salary and purpose of travel

- Self-employed applicants must provide a trade license

- Taiwanese documents must include a translation into English or Czech

3. Minors traveling without a parent/legal guardian 

- Written consent from a parent/legal guardian and copy of their passport

4. Applicants who are not Taiwanese passport holders 

- submit ARC residence card + one copy (The card must be valid for at least 3 months after departure from the Schengen area)

 

Additional Notes on Applications: 

- Taiwanese documents must be submitted with a translation into English or Czech

- Each applicant must submit a separate application

- Family members of EU citizens. The above requirements do not apply to family members of EU citizens, who must submit with their application the original document proving their relationship to the EU citizen (most commonly a birth certificate or marriage certificate), a copy of the EU citizen´s passport, and proof that the applicant is accompanying or joining the EU citizen in the Schengen area.
