Minors must produce an unabridged birth certificate reflecting the particulars of both parents of the child. If he or she is not travelling with both parents, then also consent in the form of an affidavit from parents or the other parent. 

São Tomé and Príncipe - now the Embassy in Lisbon is accredited

Please note that on 22 April 2015 the Embassy of the Czech Republic in Lisbon took over the diplomatic, consular and visa responsibility for São Tomé and Príncipe from the Embassy of the Czech Republic in Pretoria. 

Embassy in Pretoria accredited to Madagascar

Please note that on 24 February 2015 the Embassy of the Czech Republic in Pretoria took over the diplomatic, consular and visa responsibility for Madagascar from the Embassy of the Czech Republic in Addis Ababa.
