1/ The applicant, as a holder of temporary protection, must appear in person at the Embassy in London after making a prior appointment.

2/ The Embassy will verify the applicant’s identity on the basis of their travel document and proof of temporary protection.

3/ The applicant will complete two copies of a form provided at the Embassy.

4/ The Embassy will hand one copy to the applicant and send the other to the Ministry of the Interior of the Czech Republic.

It is also possible to renounce temporary protection in the Czech Republic without personal attendance. In such cases, the applicant’s signature on the form must be officially certified.
The completed form must be sent by post (not by email) to the Embassy in London.
The second original copy remains at the Embassy for collection and is not sent back to the applicant.

The Embassy accepts applications for renunciation of temporary protection from all Ukrainian citizens, regardless of their place of residence in the United Kingdom (i.e. without a territorial jurisdiction requirement).

Health Insurance

It is necessary to fulfil the notification obligation to the health insurance company with which the foreign national was registered in the Czech Republic.

Persons insured with VZP may report this by email to info@vzp.cz by sending a scanned copy of the document Declaration of Renunciation of the Right of Residence for the Purpose of Temporary Protection.

Health insurance coverage will be terminated as of the date on which the visa was cancelled.
