How to Submit an Application

If you are unable to reach us by phone (which may occur due to a high volume of calls), please send us an email including your phone number so that we can contact you. Please also indicate what documents you have available and what you are applying for.

Note: The Embassy processes around 100 emails per day. We will do our best to respond as soon as possible, but replies may not be immediate.


Required Supporting Documents

The key document for registering a death that occurred in the United Kingdom in the Czech registry is the UK death certificate.

This document must first be legalised with an apostille issued by the UK Foreign, Commonwealth & Development Office (FCDO).

It must then be officially translated into Czech, including the apostille. Both the original document and the apostille must be translated, and the translation must be securely attached to the documents. Translations prepared by the applicant are not accepted.

Only the following types of translations are acceptable:

  • ● If the translation is prepared by a translator based in the UK (bearing a UK translator’s stamp or a Czech stamp that is not from a court-appointed Czech translator), the Embassy must additionally certify the translation for use in the Czech Republic. This certification is subject to a consular fee.
  • ● If the translation is prepared by a Czech court-appointed (sworn) translator (identified by a round stamp bearing the Czech national emblem), no further certification by the Embassy is required. Such translations can also be arranged in the UK (see the list of translators).

Important notice:
The Embassy has recently recorded an increase in cases where private intermediaries offer apostille and translation services. When reviewing documents obtained through such providers, a number of issues have been identified, including:

  • ● apostilles issued for incorrect documents
  • ● translations lacking proper certification
  • ● translations prepared in Slovak instead of Czech

For these reasons, we strongly recommend arranging the apostille independently and using authorised translators.


Documents Required

To obtain a Czech death certificate, the following documents must be submitted:

  • ● Completed Form No. 9 – Death Registration Record
  • ● Proof of the deceased’s Czech citizenship
  • ● Original UK death certificate in full form, with apostille, together with its Czech translation

If available, please also provide:

  • ● Birth certificate of the deceased (and for married or divorced women, also their Czech marriage certificate)
  • ● Birth certificate (if the deceased was single)
  • ● Marriage certificate (if the deceased was married)
  • ● Health insurance card or proof of the Czech health insurance provider with which the deceased was registered
  • ● Certificate of burial or cremation
  • ● Medical report (death certificate / report of examination of the deceased), including a written notice from a doctor if a medical or forensic autopsy was performed
