Applicants for a long-term visa or permit must register to submit an application via the email jakarta@mzv.gov.cz. Registrations submitted in any other way will not be considered.

The following information must be included in the registration email:

• Name of the applicant
• Date of birth
• Passport number
• Purpose of the applicant's stay
• Contact details (phone, e-mail)

The registration email must include the following attachements:

• A copy of the data page of the applicant's travel document with a photo and personal data (pdf format)
• A copy of a page of a document proving the purpose of the stay in Czechia (pdf format)

If the applicant is not a citizen of the Republic of Indonesia, the Sultanate of Brunei or East Timor, s/he is entitled to submit an application for a long-term visa/residence permit at the Embassy of the Czech Republic in Jakarta only if, in addition to the above-mentioned requirements, s/he also attaches a document proving his/her residence permit in the specified countries (pdf format) in the registration email. The aforementioned document must prove continuous residence of the applicant in the country for at least 2 years.

After receiving a registration as specified above, the Embassy will send the applicant an appointment for submitting an application for a long-term visa or an application for a residence permit.

The appointment date cannot be changed.

In the event of cancellation of the assigned date by the applicant, the Embassy may set a new appointment date only after receiving a new registration.
