DUPLICATES - Czech Birth, Marriage and Death Certificates

 

Consular District of the Consulate General in New York

Please be informed that an application can only be submitted to the Consulate General in New York if the applicant resides in one of the states within the consular district: Connecticut, Delaware, Maine, Massachusetts, New Hampshire, New Jersey, New York, Pennsylvania, Rhode Island, Vermont. Residency in the consular district can be proven by a copy of an ID with the listed address or, for example, a utility bill from the place of residence.

Czech citizens are obliged to register births, marriages and deaths that occurred outside the Czech Republic with the Office of Vital Records in Brno, Czech Republic. This can be done via our Consulate. The registration of the above-mentioned events must be done as soon as possible after they occur.

The duplicates can be issued only for registry events that have already been legalized with the Czech registry.

 

 

Duplicate of a Czech Registry Documents

To proceed the duplicate of the birth, marriage or a death certificate you have to present:

1. completed Application for Duplicate of the Czech Registry Document (DOCX, 24 KB)
- your signature on the application form must be verified by a notary public if the application is not submitted personally at the Consulate
2. copy of your passport or drivers license (copy of your ID must be verified by a notary public)
3. in case you are not a relative, enclose an explanation letter why do you need the duplicate of the birth certificate

SUBMITTING THE APPLICATION
 

1. By mail

If you are submitting the application by mail, the signatures on the forms and copies of the submitted documents must be notarized. 

We highly recommend including a cover letter with basic information – address, contact details, what you are requesting from the consulate, whether additional applications for civil registration events or a passport will follow the certificate request, and a list of documents attached to the letter, etc.

If you would like us to return the original documents to you (and you will not be picking them up during a future visit to the Consulate), you must include a prepaid return envelope.

 
2. In person

You can submit the application in person at the Consulate by appointment during office hours. Please schedule an appointment via email at newyork.consulate@mzv.gov.cz.

Opening Hours

Please note that if the applicant does not complete the forms before the appointment, the application cannot be accepted, and another appointment will need to be scheduled.

When submitting in person, notarized copies and signatures are not required, as the Consulate will confirm that they have seen the original documents and will automatically verify your signature, which you will provide in the presence of the consular officer.

Appointments are scheduled in the order in which emails requesting appointments are received.

 

FEE

The current fees vary each month based on the exchange rate and can be found here:

If you need an apostille for the Czech duplicate, include extra fee for the apostille mentioned in the list of fees (field 150e)

The fee can be paid by:

  • *Cash
  • *Money order (payable to "The Consulate General of the Czech Republic in New York, 321 East 73rd Street, New York, NY 10021"
  • *Credit card during an in-person visit, except for American Express cards.


PROCESSING TIME

Once the completed application and all documents are submitted, our office forwards them to the relevant authorities in the Czech Republic. Processing takes approximately 2-3 months from the time the complete application is sent to the Czech Republic. In cases where archival research is required, the processing time may be longer.

Once the duplicate is issued, the applicant will be contacted by email. The certificate can be sent by mail (the applicant must provide a prepaid envelope) or picked up in person when arranging for other documents or a Czech passport (an appointment must be scheduled in advance).
