Czech citizens are obliged to register births, marriages and deaths that occurred outside the Czech Republic with the Office of Vital Records in Brno, Czech Republic. This can be done via our Consulate. The registration of the above-mentioned events must be done as soon as possible after they occur.

Duplicate of a Czech Birth Certificate

To proceed the duplicate of the birth certificate you have to present:

1. completed APPLICATION FOR DUPLICATE OF BIRTH CERTIFICATE (PDF, 109 KB) (your signature on the application form must be verified by a notary public)
2. copy of your passport or drivers license (copy of your ID must be verified by a notary public)
3. in case you are not a relative, enclose an explanation letter why do you need the duplicate of the birth certificate
4. fee

• the fee for issuance of the duplicate of the birth certificate
• the fee for issuance of an apostille please add extra

The General Consulate of the Czech Republic is accepting only a cash or  money order as a form of payment of the fee. The General Consulate of the Czech Republic is not accepting personal checks or credit cards.

Application for a duplicate of a Czech Marriage Certificate

 To obtain a duplicate of a Czech Marriage Certificate please send us the following:

1. completed APPLICATION FOR DUPLICATE OF MARRIAGE CERTIFICATE (PDF, 209 KB) (your signature on the application form must be verified by a notary public)
2. copy of your passport or drivers license (copy of your ID must be verified by a notary public)
3. in case you are not a relative, enclose an explanation letter why do you need the duplicate of the marriage certificate
